Independent Prescriber Pharmacist

Location: West Yorkshire (BD1)

Salary: Depending on experience

We have multiple positions for Independent prescriber Pharmacists:

Current positions:

* PCN Roles:* 37.5hr Full time roles
* 22.5hr roles
* 15hr roles





* Non PCN Roles:* 20hr role
* 40hr role
* 37.5hr role





The Role:

You will be an experienced clinical pharmacist who acts within their professional boundaries to work and develop within a team of pharmacists. You will work as part of a dynamic, multi-disciplinary team, taking responsibility for areas of chronic disease management and undertaking clinical medication reviews. Some pharmacists will work as independent prescribers.

Duties:

* Provide specialist knowledge and advice on pharmaceutical matters, including cost-effective prescribing and clinical guidelines for disease treatment, and to provide evidence-based education and training for colleagues in partnership with the practice.


* Complete discharge and clinic letters


* Provide structured medication reviews


* Provide advice and support for patients with self-limiting conditions


* Work to improve communication about medication-related issues between the practice and other care providers


* Lead on implementation of safe and efficient repeat prescribing systems




* Support practice achievement of key Impact and Investment Fund and QOF targets as required.




The ideal candidate will have:

* Previous general practice experience.




* Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council


* Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role


* Qualified or enrolled on CPPE Clinical Pharmacist Training pathway (although this will be offered for PCN roles)


* Experience in supporting common acute and long-term conditions


* Knowledge, awareness and experience of brief intervention and referral pathways that provide opportunistic advice and support for preventable illness and/or long-term conditions receptive to lifestyle/pharmacological support




* Strong clinical leadership, communication, and interpersonal skills


* Some roles require you to be an independent prescriber.
